Patient Safety Culture Reports

Improving patient safety one organization at a time

This is where most organizations fall over – either by creating complex dashboards that nobody understands or by creating a one size fits all report that doesn’t have enough detail. Our experience has taught us that healthcare professionals don’t want to focus on anything but providing care, so our job is to support them with the information in a form they understand and need – from “board to ward” we provide reports at just the right level of detail.

The raw data from the survey itself is extremely complex to understand – a dashboard with another user name and password is something which executives and front line staff have no time to be looking at. From our experience organizational level and individual reports have been proven to grab the most attention.

The organizational report will be suitable for presenting to C-Suite, Executives and the Board. The Executive Summary will be of particular interest and provide a very polished view of of the current state with details being included in the rest of the document for anyone wanting a deeper dive.

Item by Item

In the body of the report there will be a breakdown of the ten composites and their positive scores. This provides a great overview but we have found executives start asking about individual items from within the report. They really want the details of what contributed to the score; especially since each composite is made up of 3-5 questions.

So we provide them with this detail in an easy to understand form. Each item is presented with its score contrasted with the AHRQ average, thus providing an indication of exactly which question contributed and how we compare in that very specific question to other hospitals.  Understanding high scores is just as important as identifying areas of improvement- our approach to action planning works towards your strengths and ensures you celebrate and spread successful practices.

Variation and Focus

From our experience we found that each survey dimension needs to be represented visually so readers can easily discern how well their segment / area is doing compared to others in the organization. Understanding the variation in scores is key to determining where resources need to be assigned. Healthcare staff are unlikely to drill down using complex interactive analysis tools so we did the hard work for them and created simple heat-maps which are easy to understand.
